First e-book, not well versed in computers, have pics stored in Picassa, Narrative in word
Not sure your document would qualify as an e-book but
go to beginning of doc - add a blank page - and type or insert picture for cover. Locate pics in picassa and add them to word . Lastly save as pdf
BTW if you have chapters in your book - its super easy to create a table of contents
